    The patch to the settop app appears to have been involved.

    I managed to resolve it by flashing 4.1.1 then adding 4.0.9utc over the old version, will add that prior to adding the 4.0.9utc patch the box was stuck on custom firmware v4.1.1 even after a couple of hard resets! Actually thought I killed it 😳

    It only affected users who had previously installed the settop patch.
